below is the Questionnaire that was sent out to Columbine Students,
and printed in a local paper. This page was set up to show what it
looked like. If you are a Columbine student or area Resident, and you
wanted to give input (and did not recieve the original), could print
this out and send it in.



Columbine High School Memorial Committee
Questionnaire
Introduction:
The Columbine Memorial is in the early stages of gathering community input for a permanent
memorial to the columbine tragedy. This public opinion questionaire is the first of several planned
opportunities for input. Please take this opportunity to help Guide the Memorial Committe in establishing
priorities and design criteria for a permanent memorial. Your responses will be valuable tools to help guide
and direct the committee's activities.

Directions:
Please mark the appropriate as instructed. For questions 2, 3A, 3B, 4, we encourage you to rank
your answers in order of preference (1, 2, 3, etc). Please return the compleated survey to: Columbine Memorial
